Extrastent Plaque Modification With Knuckle Wire for Resistant Stent Underexpansion

چکیده

Stent under expansion is one of the reasons compromising outcomes percutaneous coronary interventions. The most common cause stent calcium deposited in atherosclerotic plaque. Adequate plaque modification paramount before placement a stent, as options to handle expanded are limited and less effective. In this case report, we explore usage knuckle wire for extrastent modification, after being failed with routine methods handling expansion.

Contribution of stent underexpansion to recurrence after sirolimus-eluting stent implantation for in-stent restenosis.

BACKGROUND We used intravascular ultrasound (IVUS) to evaluate recurrence after sirolimus-eluting stent (SES) implantation treatment of in-stent restenosis (ISR). METHODS AND RESULTS Forty-eight ISR lesions (41 patients with objective evidence of ischemia) were treated with SES. Recurrent ISR was identified in 11 lesions (all focal); repeat revascularization was performed in 10. These were co...

Aggressive plaque modification with rotational atherectomy and cutting balloon for optimal stent expansion in calcified lesions

OBJECTIVE To evaluate the factors affecting optimal stent expansion in calcified lesions treated by aggressive plaque modification with rotational atherectomy (RA) and a cutting balloon (CB). METHODS From January 2014 to May 2015, 92 patients with moderate to severe coronary calcified lesions underwent rotational atherectomy and intravascular ultrasound imaging at Chinese PLA General Hospital...

Very late stent thrombosis with bare-metal stent: identifying severe stent malapposition and underexpansion by intravascular ultrasound

A 60-year-old man with a history of implantation of two bare-metal stents 2 years prior presented to the emergency department with new-onset chest pain. He has been regularly taking angiotensinconverting enzyme inhibitors, beta blockers and aspirin. Aspirin was suspended for 10 days prior to the current hospitalization in order to perform surgery to remove a kidney tumor. He underwent coronary ...

Subacute Drug-Eluting Stent Thrombosis Caused by Stent Underexpansion: Evaluation by Optical Coherence Tomography

We present the case report of a patient presenting with ST segment elevation myocardial infarction due to a subacute drug-eluting stent trombosis within the proximal segment of the left circumflex artery (LCX). Six days before a total chronic occlusion was treated at the mid segment of the LCX by overlapping two drug-eluting stents. Optical coherence tomography (OCT) was helpful to demonstrate ...
